Referring Doctor Forms

If you are a PCP and would like to refer one of your patients to see us, please fill out the form above and fax it to us at 252-222-5705 along with all recent records pertaining to the reason for referral. We will schedule the patient for our first available appointment and inform you of that date and time. 
Please note that we are no longer accepting bone density referrals.

We are no longer accepting new Medicaid patients. We are now accepting new Medicare and Tricare patients.

Fibroscan Referrals
